Once you have downloaded the form, you can open it by
going through Windows Explorer and double clicking on
ECHO2000.exe. (Note: Download time may vary based
on your Internet connection speed.) |

For viewing purposes--we recommend you change the zoom
level depending on your preferences (i.e. 75% to 100%).

For multiple entries, we recommend you make copies of
this form before entering any data. Simply by selecting
file SAVE AS from the menu and then save to another
location or with a different file name (i.e. ECHO1, ECHO2,
etc). |

Also, we recommend you print a blank copy of this form
and collect the required information before completing
the form entirely. However, this form contains validations
that require you to complete the page entitled "Company
Information" before you can SAVE or
EXIT the form. Once you have completed this
page, you can then save it and go back at a later date
for editing. |

To enter data into a text field click on the field.
To move to the next field press the TAB key; to
move to the previous field press the SHIFT TAB
key. |

To select a check box, simply place your mouse in the
check box and click. Some of the check boxes require single
and/or multiple selections. Check boxes may be grouped;
this means that only one can be selected. Selecting one
check box automatically deselects the current checked
box in the group. |

Please do not try to submit your entry application
electronically via e-mail to the DMA, they will not be
accepted. |

You must submit a printed and electronic copy on disk
of your completed entry form. The diskette must be 3 1/2-inch
(1.44 MB - IBM formatted). Both the hard copy and disk
must be sent in the same package along with your entry
fee and two creative samples to the DMA ECHO Awards for

All entries postmarked after April 10 are subject to
a late fee of $75.00 and entries will not be accepted
after April 17th. No Exceptions! Please do not call to
